Its slow and I have 51 process running and at idle its using 43% CPU usage

According to the screen shot you're showing a system idle process of 83%. While this doesn't match a 43% CPU usage it does appear that 17% is being taken up by something inside System. It would appear that something is running that is being hidden.

First question: PC or Laptop?

On my XP machine I have 56 running processes and that includes AOL 9.1 software, but when Idle I have 0% CPU usage. System Idle will show 99% as it should. Not too shabby in my book.

Second question: Is this a Vista machine? The symptoms have been indicative of the Vista OS for me and others.

Third question: Also, have you installed anything new lately? A new hard drive perhaps?

My research on this thus far indicates a possible driver issue. If you haven't already done it be sure to check Device Manager to see if there are any flagged items, update everything one at a time to see if the problem goes away (be sure to give it over an hour to settle after each update that might have signaled a fix just to make sure it doesn't return).

Fourth question: Have you had any recent installs that, while not appearing to be part of the particular software, may have included a driver for something you don't use?

Another note is check the inside for any damage to any USB ports. If any of them are not functioning or appear fried be sure to disconnect them totally.

This appears to be an old thread (relatively) but how many of those processes are from your ISP? I see several that seem unnecessary. Seems like someone said that you have had your machine worked on recently so I assume this is fixed. If not, get with me and we can hammer out what is slowing you down.
